How the service works
Hospitals can access the service in two ways. The discharge team can refer the patient to Care Sourcer’s matching team by telephone, or use the technology platform themselves from within the hospital.
1. Discharge team refer patient to Care Sourcer
Care Sourcer’s expert team and technology platform take the burden away from hospitals. Discharge teams can call a dedicated phone number and Care Sourcer will find care for the patients within 24 hours.
This is an ideal entry point into the Care Sourcer service, with no financial risk and minimal contractual requirements.

There is no cost to register, no obligation to use the service. Each successful discharge is charged at £650.
2. Discharge team use platform directly
Care Sourcer’s web-based platform is directly accessible by the discharge team, enabling healthcare professionals to quickly source care themselves while still meeting outcomes.
The process is fully streamlined, from the initial detailing of care requirements, to contracted framework providers, through to the collation of offers and finalising of contracts.
A tiered pricing model which includes direct access to the Care Sourcer platform.
This option can also provide valuable market insight allowing CCGs and Local Authorities to accurately plan for the future and provide the appropriate care settings, based on locally attributable data points.
