Case study – Gloucestershire Hospitals NHS Foundation Trust
Background
Care Sourcer worked in partnership with Gloucestershire Hospitals NHS Foundation Trust over the winter months of 2017/2018.
Handovers were made from a NHS discharge team member to a Care Sourcer team member, who was then responsible for all patient, family and provider brokerage, including home visits and assessments.
Once all logistics and considerations had concluded, Care Sourcer handed them back to the discharge team to arrange for discharge on the agreed date and time.
Results
Over the six-month winter period, Care Sourcer was referred 81 patients who on average were discharged within five days of the case being handed over to the Care Sourcer matching team.
Throughout the six month period, Care Sourcer achieved a 40% reduction of delayed transfer of care and estimated a saving to the Trust of £61,512.50 with wider hospital benefits.
An independent economic assessment of this partnership identified an overnight saving versus existing brokerage services of £25m, with a further potential saving of £277m if the Care Sourcer care navigation service was consistently used across the NHS.
