DCSS will open and maintain a file in respect of each Service User and the file shall contain the following:
- The Care Plan
- Key points of Communication for the Service User
- Key points of Communication for the Provider
- Comprehensive Risk Assessment completed by Provider
- Review form
- Annual review form
- Correspondence records of communication relating to Service Users
- Communication log for all services provided by Employees at each visit that is to be read, completed and signed by the Employees attending
- Medical Administration Record (MAR) sheet, if required
File shall be available to the Commissioner and will form part of the service review / contract management process.
